New Passport

• The applicant has never previously held a U.S. passport.
• The applicant has not had a U.S. passport issued in the past 15 years.
• The applicant was under 16 years old when their most recent passport book was issued.
• The applicant is a U.S. citizen and over 18 years of age.

Passport Renewal

• Applicant has most recent passport in their possession and should not be reported lost, stolen or damaged

• Applicant was 16 years or older when passport was issued
• Applicant’s passport was issued within the past 15 years
• Applicant’s passport must not be damaged

Child Passport

• Applicant is a US citizen under 16 years old.
• Applicant has never held a U.S Passport Before.
• Applicant has been issued a passport and it is going to expire soon or it has already expired.
• Applicant lost or had their passport stolen

Lost / Stolen / Damaged Passport

• Applicant is a US citizen over 16 years old
• Applicant is unable to locate his or her passport
• Applicant’s passport was stolen or damaged

Second Passport

• Applicant has a valid US passport is in his or her possession
• Applicant was 16 years or older when passport was issued
• Applicant has more than 4 blank pages on their current passport

Change or Correct Name

• Applicant must have the US passport is in his or her possession
• Applicant must have legally changed their name since most recent passport was issued
